The
financial creditors shall submit their proof of claims by electronic means only.
The operational creditors (including workmen and employees), statutory authorities and any other creditors may submit their proof of claims in person, by post or through electronic means.
The proof of claims is to be submitted by way of
sign and filled forms along with declaration and documentary proof in support of the claim, as prescribed under Insolvency and Bankruptcy Board of India (Insolvency Resolution Process for Corporate Persons) Regulations, 2016.
Submission of false or misleading proofs of claim shall attract penalties.

You are required to submit the scan of the signed and printed form with an declaration and documentary proof in support of your claim to the IRP by: (i) electronic means only post (if you are a financial creditor), and (ii) in person, by post or by electronic means (if you are an operational creditor or any other creditor), at the address mentioned above.
